Free and Reduced Price Meals:

Centralia School District participates in the Free and Reduced Meal Pogram and the Direct Certification Program through through the State of California.

A student is automatically certified to receive free meals if their household currently receives Food Stamps (FS), or if he/she receives:

• California Work Opportunity and Responsibility to Kids (CalWORKs)
• Kinship Guardianship Assistance Payments  (Kin-GAP)
• Food Distribution Program on Indian Reservations (FDPIR) benefits

If a child is not direct certified for free or reduced price meals, the parent/guardian may apply for their child.  Applications are available at all school offices and the District Office.  See the Income Eligibility scale for qualifying information.

FOR SCHOOL YEAR 2010/2011

Please complete and sign the application, and return it to the District Office at 6625 La Palma Ave. Buena Park, CA 90620.

Applications CANNOT BE PROCESSED and will be returned to the applicant if the following information is missing:

•·   The name of the child or children for whom you are applying for free or reduced-price benefits.

•·   The names and income of all other household members.

•·   The signature of the child's or children's parent or guardian.

•·   The Social Security number of the person who signed the application. If the person signing the application does not have a Social Security number, write "none" in the space provided.
